As used in this agreement, "OLIVE TREE" refers to THE OLIVE TREE COMMUNITY CENTRE, a not-for-profit organization, and its officers, directors, employees, assigns, and agents (including any third party designated and approved by the OLIVE TREE, such as individuals or entities involved in print, publication, television, broadcast, or video media). "Participant" refers to any volunteer or individual involved in an OLIVE TREE event or activity.

In consideration of my participation in any OLIVE TREE outreach, meeting, or event (collectively referred to as the "Event"), I agree to the following terms:

I understand that participating in an Event may involve certain risks, including but not limited to bodily injury, illness, property damage, and other potential hazards. I acknowledge that these risks may arise from various factors such as physical activity, environmental conditions, or interactions with others. I voluntarily assume full responsibility for any risks associated with my participation.

Except in cases of gross negligence or willful misconduct by the OLIVE TREE, I hereby waive any claims or causes of action that I may now or in the future have against the OLIVE TREE arising from my participation in the Event. I further agree to indemnify and hold harmless the OLIVE TREE from any and all claims, demands, liabilities, damages, costs, or expenses (including legal fees) resulting from my involvement in the Event.

In the event that I sustain an injury or illness during the Event and am unable to provide consent, I authorize the OLIVE TREE to administer, or arrange for the administration of, necessary first aid or medical treatment. This may include treatment by a physician or hospital selected by the OLIVE TREE. I accept full responsibility for any medical expenses incurred.

I grant the OLIVE TREE permission to capture and use photographs, video, or audio recordings of me taken during the Event for promotional, educational, or fundraising purposes. I waive any right to inspect or approve the finished materials and release OLIVE TREE from any claims related to the use of my likeness.

This agreement shall be binding upon my heirs, personal representatives, and assigns. It shall be governed by and interpreted in accordance with the laws of the Province of Alberta and the Province of Saskatchewan. This agreement represents the entire understanding between myself and the OLIVE TREE regarding this subject matter and supersedes any prior agreements, whether written or oral.

By signing below, I confirm that I have read this agreement, fully understand its terms, and voluntarily agree to be bound by them. I understand that this form involves a release of legal rights.
